Liability Insurance Individual Payments

Please read carefully before continuing.

As part of participating in the league, each player is required to pay a $35 fee for individual liability insurance. Please note, this is NOT medical insurance and will not cover any injuries or medical expenses related to your participation in games or practices.

This liability insurance is designed to cover any damages caused by the park or its facilities to the player. It ensures that, in the event of an accident involving park equipment or conditions, there is protection in place for liability purposes.

In order to participate in the UFL, every player must pay the $35 liability insurance fee. If this payment is not made, unfortunately, you will not be able to take part in the league. If you have any questions or need to discuss possible adjustments, please contact your team captain or owner for clarification.

WAIVER AND RELEASE OF LIABILITY

THE ULTIMATE FUTBOL LEAGUE LLC

2026/27

IN CONSIDERATION OF the risk of injury that exists while participating in THE ULTIMATE FUTBOL LEAGUE LLC (hereinafter the “Activity”); and

IN CONSIDERATION OF my desire to participate in said Activity and being given the right to participate in same;

I HEREBY, for myself, my heirs, executors, administrators, assigns, or personal representatives (hereinafter collectively, “Releasor,” “I” or “me”, which terms shall also include Releasor’s parents or legal guardian if Releasor is under 18 years of age), knowingly and voluntarily enter into this WAIVER AND RELEASE OF LIABILITY and hereby waive any and all rights, claims, or causes of action of any kind arising out of my participation in the Activity.

1. SEASON AND EFFECTIVE PERIOD

This Waiver applies specifically to THE ULTIMATE FUTBOL LEAGUE LLC – Season 2026/27, First Semester, commencing on October, 2026, and shall remain in full force and effect for the entire duration of my participation in said season, including all games, practices, events, promotional activities, and related activities.

2. RELEASE OF LIABILITY

I HEREBY release and forever discharge THE ULTIMATE FUTBOL LEAGUE LLC, located at 133 NE 2nd Ave, Miami, Florida 33132, and its affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors, and assigns (collectively, the “Releasees”) from any and all physical or psychological injury that I may suffer as a direct or indirect result of my participation in the Activity.

I AM VOLUNTARILY PARTICIPATING IN THE ACTIVITY AND I AM PARTICIPATING ENTIRELY AT MY OWN RISK. I AM AWARE OF THE RISKS ASSOCIATED WITH PARTICIPATING IN THIS ACTIVITY, INCLUDING BUT NOT LIMITED TO: physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), economic or emotional loss, and death.

3. ASSUMPTION OF RISK

I understand that these injuries or outcomes may arise from my own negligence, the negligence of others, conditions related to travel to and from the Activity, or from conditions at the Activity location(s). I knowingly and freely assume all such risks, both known and unknown.

4. INDEMNIFICATION

I FURTHER AGREE to indemnify, defend, and hold harmless the Releasees from any and all claims, suits, actions, liabilities, damages, costs, or expenses (including attorney’s fees) brought by me or on my behalf.

5. MEDICAL AUTHORIZATION

I authorize THE ULTIMATE FUTBOL LEAGUE LLC to provide or arrange emergency medical care deemed necessary, including first aid, CPR, AED use, emergency transport, and disclosure of medical information to medical personnel. I agree to assume full financial responsibility for all related costs and acknowledge that I am responsible for carrying my own health insurance.

6. MEDIA, IMAGE, AND INTELLECTUAL PROPERTY RIGHTS

I ACKNOWLEDGE AND AGREE that any and all photographs, videos, graphics, recordings, images, likenesses, voice recordings, or other media captured or created during my participation in the Activity are the sole and exclusive property of THE ULTIMATE FUTBOL LEAGUE LLC.

I HEREBY GRANT full, perpetual, irrevocable, worldwide, royalty-free rights to THE ULTIMATE FUTBOL LEAGUE LLC to use, reproduce, edit, distribute, publish, display, and otherwise exploit such materials without any further permission, approval, notice, or compensation to me.

NO PLAYER PERMISSION IS REQUIRED for the use of any such images or materials.

7. NO SPONSORSHIP OR MONETARY RIGHTS

I UNDERSTAND AND AGREE that participation in THE ULTIMATE FUTBOL LEAGUE LLC does not grant me any sponsorship rights, financial compensation, revenue sharing, or monetary benefits of any kind related to logos, branding, promotional materials, social media posts, broadcasts, or marketing content, regardless of whether my image or likeness appears in such materials.

8. DISCIPLINARY CODE AND SANCTIONS

I ACKNOWLEDGE that I am required to know, understand, and comply with the League’s Disciplinary Code of Conduct, available at www.theufleague.com.

I AGREE that I am subject to all disciplinary rules, decisions, fines, suspensions, or sanctions imposed by THE ULTIMATE FUTBOL LEAGUE LLC in accordance with said rules, without refund or compensation.

9. GOVERNING LAW

This Release shall be governed by and interpreted in accordance with the laws of the State of Florida, without regard to conflict of law principles. This Release supersedes any and all prior oral or written agreements.

10. DAMAGE TO PROPERTY

I agree to be financially responsible for any damage to equipment or facilities caused by my willful actions, negligence, or recklessness, including costs of repair or replacement.

11. SEVERABILITY

If any provision of this Release is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

12. EMAIL COMMUNICATIONS AND MARKETING AUTHORIZATION

I AUTHORIZE THE ULTIMATE FUTBOL LEAGUE LLC to store and use the email address provided in this registration to send league-related communications, including but not limited to season updates, schedules, results, announcements, events, partner or sponsor promotions, merchandise, offers, and other marketing communications related to the League.

I understand that I may unsubscribe from marketing communications at any time, while still receiving transactional or administrative emails related to my registration, payments, waiver, league participation, discipline, safety, or other essential league operations.

13. ACKNOWLEDGMENT AND SIGNATURE

I, THE UNDERSIGNED PARTICIPANT, AFFIRM THAT I AM 18 YEARS OF AGE OR OLDER, THAT I HAVE READ AND FULLY UNDERSTAND THIS WAIVER AND RELEASE OF LIABILITY, INCLUDING THE EMAIL COMMUNICATIONS AND MARKETING AUTHORIZATION, AND THAT I AM SIGNING IT FREELY AND VOLUNTARILY. I UNDERSTAND THAT THIS IS A LEGALLY BINDING CONTRACT AND A RELEASE OF LIABILITY THAT CANNOT BE MODIFIED ORALLY.
